申請人必須在註明(*)之欄目，提供所需的個人資料。如未能提供，校方未必能處理有關入學申請。
根據《個人資料（私隱）條例》，本申請表內有關個人資料將被用於回應、跟進、溝通和處理您的申請或其中 您提出的任何疑問、意見或要求。在未得到申請人的同意之前，校方不會向第三者披露或轉移所收集得關於你的資料。
如入學申請不被接納，校方將銷毀一切有關個人資料；成功申請者個人資料將會存入學生檔案中，並由校方保管。
根據《個人資料（私隱）條例》，申請人有權查閱及更改填報於申請表內的資料，並可索取此等資料的副本。申請人如須查閱或更改其個人資料，請向本校校長提交書面申請。
本校可能會透過家長的聯絡資料推廣校本服務，包括通知與學校相關的活動及訊息等。收集的個人資料有可能用於彙編統計數據來評估和審查入學計劃。家長可隨時選擇取消接收學校發送的宣傳資料。

「聲明」

本人理解學校有權根據提交的順序、幼兒年齡和其他因素進行分配學位。
本人保證此申請表上的資料是真實和準確的。我明白學校有權要求進一步的文件並可以聯繫申請人之前的學校索取相關信息。
本人了解學校所有活動將受到合理謹慎的監督，學校不會對學生及其財物的任何人身傷害、意外、損失及損毀負責。
本人進一步同意承擔我的孩子參加課堂內/外的任何活動而導致的任何人身傷害、意外、財產損失或損害，對學校及其員工、僱員、代理人豁免責任，免除懲罰，進行辯護並永久放棄追索所有責任、訴訟、要求、損失、開銷、花費、行動和法律行動。
本人明白，除非家長以書面通知校方，否則學校日常教學中可能會為我的孩子拍照、錄影、錄音，用於定期的評估報告、社會媒體或學校的網頁及宣傳材料。少數情況下可能會用於校際交流，監控或其他教學用途。
本人明白學校樂意與家長互動、家庭和學校之間及時和尊重的溝通對孩子的成功至關重要。

政策和做法：

學校校長負責奧恩國際幼兒園暨幼稚園（簡稱奧恩）所有入學申請的審批和錄取工作。
每學年小組班和幼兒班會連續收取十一期的學費；低班和高班連續收取十期的學費。
一切已交學費概不退還。每月24日將預繳下個月之學費，學費透過任何本地銀行直接存款收取，付款授權書必須在報名登記時一併完成，並在指定的時間內提交給銀行。
若幼兒因疾病或要事請假，請假期間仍須如期繳交學費。
如因任何原因需要退學，家長須至少於兩個月前以書面形式通知學校。
若幼兒於中途入學或退學，亦須繳交該月之整個月學費（學費不能按日計算）。
奧恩幼兒園暨幼稚園的家長將享有奧恩親子天地舉辦之家長工作坊和培訓的優先權及優惠。
奧恩國際幼兒園暨幼稚園保留最終決定權。

Collection of Personal Data

Please note that it is mandatory for you to provide the personal data marked with (*). In the event that you do not provide such personal data, the kindergarten may not able to process your application.
According to the Personal Data (Privacy) Ordinance, the above Personal Data provided will be used will be used to respond, follow up, communicate and process your application or any queries, feedback or claims made by you. In this connection the data will be handled by the Mighty Oaks or affiliated companies only. The information collected about you will not be disclosed or transferred to third parties without your prior consent.
The application of unsuccessful candidates will be destroyed. The application of successful candidates will become part of the student file and this data will thereafter be handled by the kindergarten.
In accordance with the Personal Data (Privacy) Ordinance, applicants have the rights of access to and correction of the personal data contained in the application form, and the right to request a copy of such data. Applicants wishing to access or make corrections to the data should submit written requests to the School Principal.
The contact information collected may be used to provide you with information regarding future school-related opportunities and communications as set out below with regard to direct marketing. It may also be used to compile statistics to review and evaluate the admission programme. Parents can easily unsubscribe any unwanted email/direct marketing material at any time.

Declaration

I understand the school reserves the right to allocate places based on order of submission, age and other considerations.
I certify that the information on this application form is true and accurate. I am aware that the school may request further documents and may contact former schools of applicants to request relevant information.
I understand that all school-related activities will be supervised with reasonable caution and the school and its members will not be held responsible for any personal injuries, accidents, losses and damages to the students and their belongings.
I further agree to waive any and all claims I may have against, and release the School from all liability, and agree not to sue the school and its directors, managers, agents, employees, volunteers, representatives, and/or any other authorized individual for any personal injury, accident, property damage or loss sustained as a result of my child’s participation in any activities held within or outside the premise during class time, arising out of any cause whatsoever.
I understand that unless notified in writing by the parent, the school reserves the right to use photographs, video & audio recording taken of my child in regular assessments, social media pages, website or promotional materials, school communications, monitoring or other educational use.
I understand that the school welcomes parent interaction and I also understand that timely and respectful communication between home and school is vital to the success of my child.

Policies and practices:

The School Principal is responsible for the approval of all admissions into Mighty Oaks International Nursery & Kindergarten (Mighty Oaks) and is also responsible for the ongoing enrolment of all students at Mighty Oaks.
School fees are paid in 11 consecutive instalments (PN& K1) and 10 consecutive instalments (K2&K3) for each academic year.
School fees are non-refundable and collected through direct debit of any local bank in advance around the 24th of each month. A debit authorisation form should be completed at the time of registration and submitted to the bank before the time specified.
School fees need to be paid as usual during the period of your child’s sick leave or casual leave.
In the case of your child’s withdrawal from school, you must inform the school with two months’ written notice.
A full month’s worth of school fees will be charged even if your child is admitted to or leaves the school halfway through the month (school fees cannot be prorated or calculated for payment on a daily basis).
Mighty Oaks parents will enjoy priority booking and discounts on workshops and trainings organised by Mighty Oaks and affiliated companies.
Mighty Oaks reserves the right of final decision.
